Right here.
Somebody installed classified replacement breakers in the panel, so the Square D warranty rejection feature rebelled by causing wacky voltages at the bus stabs.

All QOs, and I think this would have been well before the counterfeits were discovered.

I will give everyone a hint.... take a calculator to the odd voltages and see if anything adds up.

it's obvious that the highs and lows add up to nominal but what about the 240 high on one leg?

okay 220v approx on one leg.
