When did heat tape/trace start requiring GFI?

It was because of fires, not personnel protection. Lots and lots of fires.

Ground fault for equipment. Not Class A.

Many fires in mobile homes, heat trace got crossed, overheated, melted, fire. Product standard changed to require GFCI plug, metal braid, and the GFCI outlet

Correct, there is quite a few pictures on the net of the fires. One was a ship that caught fire, the heat trace was covered with spray insulation and overheated.
