is the metering cold sequenece or hot sequence? If there is a 200A OCPD before the 2 panelboards then it doesn't matter how many boards are fed from the meter becaise the whole system will only be sized for 200A.

Hot sequence, lever bypass 200 amp socket, commercial building, 2/0 cu feeding the meter base and the two breaker panels (one inside and one outside), each having a 200 amp main. The conductors feeding the panel mains are doubled up under the meter base lugs.

The location of the service disconencts is a violation of 230.72. The two wires under the single lug is a violation unless that lug is listed for 2 conductors. The size of the wire may or may not be a violation. If there are multiple service OCPDs, the code only requires that the service conductors be large enough for the calculated load. See 230.90(A) Exception #3.
