10-foot Tap Rule 240.21(B).(1)

See the attached riser diagram. The electrical inspector said we needed main breakers on the electrical panels but NEC 240.21(B).(1) appears to state otherwise. He mentioned informational note- referencing 408.36.
Is 240.21(B)(1) allowed for every device except panelboards?
Comments would be appreciated.

The feeder tap might be legal, not sure exactly what goes to where, but you don't have proper overcurrent protection for the panelboards in question.

Unless I am seriously misreading things, we have the very unusual situation of the same question being asked by both parties in an apparent disagreement.
