We have a 480V three phase, three wire SWBD (no neutral) that is currently designed to be within three feet of concrete wall, which is condition according to 110.26(A)(1). Should this be extended to 3.5' even though there is no neutral?

We always used 3’ condition 2 for 480V Panelboards if there was no neutral

If it's ungrounded then look at the Article 100 definition of Voltage to Ground.
 
Yes.
Not having a neutral in the equipment is not the same thing as being an ungrounded system
Correct but he didn't state in the OP what the system is. If it is grounded then the voltage to ground would be between 151 and 600 volts and condition 2.
