110.27 Guarding of Live Parts

110.27(A)(4) indicates live parts of electrical equipment above 8 ft is considered guarded against accidental contact.

Since the handle of an OCPD device can't exceed 6'-7", what would type of live parts of electrical equipment would this be referring?

Any examples of where someone has seen live parts exposed like this?

I have been in transformer rooms/vaults where all of the wiring was open air conductors supported from the roof on insulators.

Utility overhead service drops are routinely found "unenclosed".
