Article 110-58 in NEC 2008 states that you need to have a switch or a circuit breaker that simultaneously opens all ungrounded conductors of the circuit "shall be within sight of the transformer"...

Are there any exceptions to this rule? (In the 2008 code)

Any installations that aren't in Tunnels???

I don't see an exception to that rule in 110.58. That section is for over 600 volt, tunnel installations.
