400.5(A) covers several things primarily "allowable ampacaties". Allowable ampacities as per ambient temps and/or number of current carrying conductors within one flexible cord or cable coupled with the correction factors from Table 310.16 as applicable. Table 400.5 speaks for itself, however, curosity compels the question, couldnt 12/4 w/grd be argued as a cable with more than 3 current carrying conductors?, or is it because it (12/4 w/grd) is not listed as a "flexible cord or cable"? or am I supposed to be derating 12/4 w/grd if and when I use it? or, ok thats enough or's, let's just get on with it.
 
1 black conductor
1 white conductor
1 red conductor
1 white w/red stripe conductor
one copper grd.
Circuit one; black and white
Circuit two; red and white/red
shared ground.
2 seperate circuits contained in one sheath.
 
As Marc said even with derating the ampacity would still be 20 amps. Now if you bundled 3 of these cables together you would see an adjusted ampacity below 20 amps.
 
I use 14 2-2 rx all the time, works great for Two afi or gfi circuits since it contains two neutrals... but I also use 14-4 for a FAN/LIGHT/NIGHTLIGHT setup... as far as derating, I think it was 7 current carrying conductors? And it was only for firecaulked holes? Whatever the # was, seemed to me it would be more than 3 cables per hole to achive this...

Don't get them mixed up: In cable, the EGC is not counted; in cord, it is. Bob's comment caught it.
