Ok, I haven't set a service in a while. I am installing a 200 amp underground meter socket and a 40 position panel. The AHJ used to make us run a #4 from the grounding bushing outside to the grounding bushing on the inside then to a ground lug inside the panel. He also required a #4 wire from the meter socket to the ground rod. Is this still alright, or how does anyone else do it?
 
Way too many variables and way too little info for definite answers.
IF you nipple between your meter socket and your panel is metallic, 250.92
will require it to be bonded. The method of bonding depends on some details but a bonding bushing on one end should suffice as far as Code is concerned... you AHJ may have stricter requirements.
A #4 to a ground rod is acceptable (perhaps overkill..but again details)
Other grounding electrodes on the premises such as the concrete encased electrode and metallic water systems would also need to be taken into consideration.
AS chat with your AHJ seems in order.
 
There will be approx. 6' from the meter socket to the panel (metallic). Would a #6 from grounding bushing to grounding bushing be ok. I live outside Chicago, they get kinda crazy here.
 
Bonding follows the table in 250.66. Generally a 200 amp service needs a #4 bond. As stated the wire to the ground rod, if it is the sole connection, only needs a #6 but Chicago may want #4 for protection. No biggie- use #4 to the rod, it wont hurt.

Are you allowed to use PVC? I bet not... if you could then no need to bond the nipple
