In our hospital project our supplier provided us distribution panels equipped with 200KA surge protection device, this is the highest level of lighting surge, based on which factor they put such rating. i.e( type of building, location of the building, height of building....................etc:?).

As per BS standard you have to do risk assessment calculation, accordingly Class of Lightning protection system will be decided.
For class-1 the maximum surge the device is subjected to is 200KA. Since this is a hospital project which has a lot of sensitive medical equipment and damage of the equipment can cause the loss of patient life.
Also this depend upon the specification requirement, check the specification also.
