200A 480V Service Grounding and bonding

I have my disconnect and meter base and panel, where do my grounding and bonding terminations begin from? Do they come from the disconnect ahead of the meter or do they come from the panel down from the meter? Thanks, Mike

Is the disconnect ahead of the meter the service disconnect, or is it just a meter disconnect required by the power company?
