2014 -- 1000v phase-to-ground or phase-to-phase?

In the changes for 1000v I find the following sections say phase-to-phase:

210.13 GFPE ... 600v P-t-P
230.95 GFPE ... 1000v P-t-P
240.2 Supervised Industrial Installation ... 300v P-t-P
240.13 GFPE ... 1000v P-t-P
280.4(A)(2) Impedance or Ungrounded System ... P-t-P

T490.24 Min Clearances P-t-P & P-t-G
555.4 Distribution System ... 1000v P-t-P
708.52 GFPE 1000v P-t-P

In numerous places it just reads 1000v.

Will I be able to use class 0 (1kV) need to go to class 1 (7.5kV)?

Does that also mean that we'll be needing 2000V DVMs?
 
Good question, the closest thing I could think of is in Article 100 definitions, not sure if it applies:

Voltage to Ground. For grounded circuits, the voltage be-
tween the given conductor and that point or conductor of
the circuit that is grounded; for ungrounded circuits, the
greatest voltage between the given conductor and any other
conductor of the circuit.
