If one considers the DW junction box as an outlet similar to a lighting outlet box then yes.

As the rule is written and with the above statement; IMO, yes the DW needs GFCI protection.

Opinions will vary.

IMO, the junction box supplying the dishwasher does meet the definition of "outlet". A hardwired or cord and plug connected dishwasher both need GFCI protection.
 
Would hard wiring with n/m cable still need to be GFCI protected or is it just recepts?

210.8(D) does not say "receptacle outlet". It only says "outlet". Both of those terms are defined in Article 100 Definitions.

As the hard wired NM cable is connected to the wiring internal to the Dishwasher, the Premises Wiring System ends and the Appliance wiring starts. The Appliance is the Utilization Equipment that is taking current from the wiring system at the point known as the Outlet.

Yes, a hardwired dishwasher is required to be GFCI protected.
 
A case could be made to clarify this article. Branch circuit or outlet would better suit circumstances although article 100 is sufficient - albiet “circuitous”.
 
The very reason it is not in 210.8(A) or (B) is that (D) requires the outlet to be GFCI protected. (A) & (B) require receptacle outlets to be protected.

What part of the circuit is the "outlet" with a cord and plug connected dishwasher ?
