225.31(B) … “The disconnecting means shall be at a readily accessible location nearest the point of entrance of the conductors.”

Just how literal should I take this? If conductors enter from the bottom of panel should I install panel so that main breaker is also located at the bottom (“nearest entrance of conductors”).?

Not that literal :)
In fact, the distance allowed once the conductors enter the building varies by jurisdiction but is normally around 3 ft to 5 ft.

There are various amendments for this.

Without an amendment I read "nearest" as you come directly into the back of the interior disconnect, or run directly up or down from the entry point into the disconnect.

Our local amendment for service disconnects specifies no more than 10' of conductor length inside the structure, but is also requires service conductors to be installed in RMC or IMC.

OP is asking about article 225 which is feeders not services.
The language relating to the location of the disconnecting means at a second building supplied by a feeder is almost identical to that for the location of the service disconnect.
225.31(B) Location.
The disconnecting means shall be installed either inside or outside of the building or structure served or where the conductors pass through the building or structure. The disconnecting means shall be at a readily accessible location nearest the point of entrance of the conductors. For the purposes of this section, the requirements in 230.6 shall apply.
230.70(A) (1) Readily Accessible Location.
The service disconnecting means shall be installed at a readily accessible location either outside of a building or structure or inside nearest the point of entrance of the service conductors.

WA state has a 15 foot limit.

Keep in mind that if the conductors are run under a concrete floor, the "point of entrance" is where they come up above the floor. If they come up directly into a panel, that counts as zero distance.
