We have had our 1st AHJ adopt 230.85 (this area has been on the '2017 NEC previously)
On 400 amp services (dual 200 amp panels) what is the most common method of addressing this requirement in your area ??

No difference. 2 -200A exterior mains, grouped, and each labeled EMERGENCY DISCONNECT. Did I misunderstand the question?

Welcome to the nightmare!
Below is an example of an installation in 2020 when no equipment was available with sectioned disconnects per 230.70 IMG_8413-2.JPG

I get NUMEROUS calls about insurance calling all Zinsco bad and people needing their mains replaced. I would just make sure the customer knows enough to call out their insurance.
Same here, and I have never came across any real peer reviewed published research that all Zinsco are bad, just hype up by 'home inspectors'.
I have herd some anecdotal experiences where they don't trip but thats all.
