240.15(B)(1) question-2008 nec

I have a question regarding this section. According to the Mike Holt Code Change Book, this section is referenced as the reason for handle ties needing to be identified no if's and's or but's.
Yet 240.15(B)(1) says with or without identified handle ties. Is this a misprint, or what am I missing?

All this means is you must use identified handle ties, not a nail through the breaker handles.

[size=+1]If[/size] you use a handle tie as required by 240.15(B)(1), it has to be identified (See Article 100 for the definition of identified). You don't have to use a handle tie unless required to use one by 210.4(B) if the multiwire branch circuit serves only line to neutral loads.

If you read 210.4(B), it seems to me to require handle ties for a common disconnecting means but not required to be common trip. 210.4(C) Exception 2 requires common trip if you are serving line to line loads with the multiwire branch circuit.

I hope this is not too confusing since the reasoning is a bit circular and it doesn't appear you can ever use a multiwire branch circuit that doesnt have either handle ties or common trip circuit breakers. :confused:
