If it is a utility-owned transformer, the secondary conductors would be service conductors, and 240.21(C) would no longer apply.

You typically get a 10 ft or 25 ft allowance for "unprotected" conductors on the secondary side, depending on which rule you follow. You then terminate at a separately-installed device with the OCPD, as close as practical to the transformer.
