Does 240.21(C )(4) still apply if the transformer is located inside the building but the secondary conductors are routed outside to the load?
 
The '17 Code added a bit of wording to clarify the for (C)(4) to apply the conductors must be outside the building except at the point of load termination.

I tend to agree with you. Essentially this means the transformer itself must be installed outdoors.
 
Depending on your transformer manufacturer and size, that often simply involves adding some rain shields
