Maybe I'm overthinking this but what exactly does the phrase "by portable means" mean?

"For overcurrent devices adjacent to utilization equipment that they supply, access shall be permitted to be by portable means."

An example would be a ceiling mounted HVAC unit with the disconnect adjacent to it and accessible by a portable;e ladder.

Portable means is basically a ladder or such.

Think of a fused disconnect that serves a ceiling mounted AHU that is located 8 feet up. You would need a small ladder to reach it.

Think of a disconnect that serves a ceiling mounted air handling unit located 9 feet up. you would need a medium size ladder to reach it.
