MLO or side mounted is okay. Most labels are now printed so you can flip them.

The breaker must always have ON on top.

Many, if not most, vertically mounted main breakers may be 'rotated' for bottom feed applications. This rotation often involves removing the breaker, swapping the location of the incoming lugs, and then reinstallation.

Yes, unless as noted, with a breaker that can be inverted. Also when breakers are used as switches, up is still on. 404.7.

The residential panels I use have main breakers where the handles moves horizontally rather than vertically. This allows for the panel to be mounted upside down.
