Found this gem today. Anything besides 250.6?

110.12(A) [Had one awhile back with a happy bird's nest crammed neatly in it. The bird was no where to be found...]
 
Ground & noodle tied together and both are bonded to can.

That recep is mounted ground up..... er.... is not rated for outside use.
 
Don't know how old the install is, but the '90 NEC (the oldest code book in my archive) requires the box & cover for the recep to be rated for wet location.

Plug the unused hole in the bottom of the disconnect.

you might consider 314.23 Supports.
whats wrong with the last fuse on the right , and is that a burn mark between B and C????
 
How about 344.46;

344.46 Bushings.
Where a conduit enters a box, fitting, or other enclosure, a bushing shall be provided to protect the wire from abrasion unless the design of the box, fitting, or enclosure is such as to afford equivalent protection.
Also, the neutral is derived from the equipment ground.
