Ground & noodle tied together and both are bonded to can.

That recep is mounted ground up..... er.... is not rated for outside use.

Don't know how old the install is, but the '90 NEC (the oldest code book in my archive) requires the box & cover for the recep to be rated for wet location.

Plug the unused hole in the bottom of the disconnect.

No need for a gfci, it's fused at 6 amps. That won't kill anybody.:roll::roll:

How about 344.46;

344.46 Bushings.
Where a conduit enters a box, fitting, or other enclosure, a bushing shall be provided to protect the wire from abrasion unless the design of the box, fitting, or enclosure is such as to afford equivalent protection.
Also, the neutral is derived from the equipment ground.

Maybe not a code issue, but our inspectors sure frown on it. The enclosure is ram setted through the back instead of the factory holes.
