3 Phase Commercial Cooling System

I need to connect a refrigerator in a meat market.
All I know is that it is a three phase, but the customer does not know of how many voltages it is.
My question is, To connect a three phase equipment, do I need to put a disconnect means between the freezer and the panel box, or just make the branch circuit and end it with an outlet close to the freezer?

is it hardwired or plug in?

Not knowing the voltage would be a big red flag to me. You'll need to wait until it arrives to see if you can safely energize it.

If it's cord-connected, the cord will act as a disconnect.

What if it is plug-in? Do we need to disconnect means between the panel box and the freezer? Also, when youre talkin about three phase means, do i need to use three breakers from panel box plus neutral cable and ground cable?

Or just the three hot cables with neutreal cable only?
Just to make a double check.

what we have is allot of unknowns here and the code is minimum safety standard..the meat markets I am familiar with, the smaller refrigeration display cases are cord connected and do not need to have a disconnect installed for them..I do how ever recommend a disconnect on the larger ones as the load can be quite high and can cause a spark if removed under load..
