When doing a safety inspection at a laundromat, the attendant complained of a problem with one of the outlets. It turns out that the installer (unknown) of a branch circuit never checked for voltage at main panel and hooked it up to the wild leg of a three phase service. He did a nice job of installing conduit and wiring but neglected to check the voltage (I guess) and put 208 volts into a duplex outlet. It was tagged out and a real electrician corrected the problem.
