Is there a means of shut off before meter required ,in a self contained 120/240 volt 3 phase service

By self-contained 120/240 three phase, I am guessing that you are referring to a delta with one leg center tapped for the 120V neutral (high leg delta).

The NEC does not require it, so what happens before the meter is entirely up to your utility and you should ask them.
There is a slight chance that your fire marshal will weigh in with local regulations too.
 
In my experience, many utilities require a disconnect on the supply side of the meter for 480V self contained sockets. YMMV.
Same here, usually only require supply side disconnect on 480 volts. This usually in cities/towns. Rural 480 volt services they never make us do that. If they are pulling such a meter their safety policy is to kill the transformer first. 99% of the time that transformer (bank) is at top of same pole the meter is mounted on and only supplies one customer.
 
The point is, this is a local requirement, not an NEC requirement. If you have someone in a position of authority saying it must be there, they are most likely correct. You can't really argue that you don't need it just because it isn't in the NEC, the NEC is a set of MINIMUM standards.

NEC tells us what we can and can't do when POCO does require such a disconnect. Ordinarily it can't contain overcurrent devices as POCO usually seals this disconnect therefore customer doesn't have access to such overcurrent devices, because of that it can't serve as service disconnecting means. You often will run into unfused disconnect --- meter socket--- fused disconnect (that is the service disconnecting means) all in same vicinity.
