314.30 D handhole enclosure covers

I dont believe i have ever seen bonding of a handhole enclosure to its metal hole, other than direct mechanical metal contact cover to metal hole.

Otherwise is an EGC required to be ran to and be bonded to the cover?
 
When the non-metallic handhole utilizes a metallic cover bonding of the cover is required. For service conductors that bonding is accomplished according to 250.92, for feeders or branch circuits bonding according to 250.96(A). For 250.92 the cover would be bonded to the neutral for 250.96(a) to the EGC with the branch circuit or feeder.
