If i have a 200 amp service on a building 120/240 volts with a 200 amp breaker disconnect on the outside of building must i run 4 wires in to the building?

Yes, and don't forget to separate the grounds and neutrals at the panel inside and don't install the bonding screw. Most new panels today are being supplied with the separate grounding bar installed but if it's not there it can be purchased separately.

You need two ungrounded conductors, and an EGC of a type recognized in 250.118, and a grounded conductor.

Yes as long as it is EMT, GRC or IMC

It is much easier to install 4 conductor cable than installing a conduit, unless your jurisdiction does not allow cable.
