40 CCT Max rule

As a Canadian sparky I'm curious how your rule on 40 cct max. panels work for American's. Here any size is permitted, I've installed numerous 60 CCT and up panels. On large houses you must end up with two or more panels? What are your thoughts on this rule and is there a good reason for it? Also does this rule apply to residential only or commercially as well ?

Okay, I will answer this without any amusing, but true stories. I install a 320 Amp meter base and either put in a 400 Amp service which has 2-200 Amp main breakers, or I split the service by installing a 225 Amp, 42 space panel, and a separate 200 Amp, 40 space panel. Any singular panel, whether commercial or residential, is restricted to 42 breakers, although I have heard of a 54-space panel that is used with oversized breakers that must be resticted to 42 devices.
