406.4 (D) replacements

I am planning on replacing old two wire receptacle in a house with grounding type while installing a new service. I plan on using a GFCI breaker to feed the entire circuit. After reading 406.4(D), I may need TR receptacles, Arc fault breaker with a GFCI receptacle at the first outlet. Is this right or am I overreacting. I can imagine explaining this to an old lady that wants an old receptacle replaced and I have to tell her that it will be almost $200 to replace a $2.00 receptacle.

(D)(4) doesn't apply until 2014. Otherwise, yeah, you're right. Also see 250.112 for a list of things that are required to have a ground no matter what.
