What are the specific code requirements for a 4160 vac 600hp motor connection box in regards to minimum size, distance between phases, clearance between box sides, front and back for the connections.

Do you have a 2002 NEC? See section 430.12, its a long section and would be best if you could look at it in the NEC.

majorty of 4160 volts motor boxes are oversized anyway to splice the cables with stress cones and covers and nec codes also useally 3 or 4 time bigger than 480 volts versions
