Has anyone heard of this type of transformer, or does it even exist? I have a job that has primary coming in at 13.2kV steps down to 416v/240v. I need to get three phase 208v. over to an ATS and generator set up, and am not sure how to achieve my 120/208v. to make everything play nice. Buck/Boost perhaps?

That is really suspect, are you sure a decimal hasn't been dropped or a zero missed?

4160V or 4.16KV is a common in plant voltage.

416V on the primary is available but its usage is rare ,especially when 480-120/208 is common.

He's talking about a 416/240V wye secondary. This is similar to a 208/120V wye but gives you 3 legs of 240 volts. (120 times 1.732 = 416). There is equipment that runs on 416V 3 phase, although not very common. Even 208V is relatively unused, as a 3 phase source. More often, it is the 3 legs of 120V that are utilized, making it easier to keep the transformer balanced. I would suspect that this is the main reason that 416/240 is used as well.

That's what they use in the UK and other countries. Australia, too, if I'm not mistaken.
