445.13 Says to size conductors at 115% of name plate. There is an exception when the generator is designed to prevent overloading. The typical residential generator has a circuit breaker. Is this the exception, therefore the conductors can be sized at 100% of name plate?

Off hand, I believe that rule is stating the conductors from the generator to the first over current protection shall be sized 115%. So in your case the manufacturer has already taken care of this

How does the size of the breaker relate to the size of the nameplate?
The specifications state the 22 KW Generac generator comes with a 100 amp breaker. My question, do I size the conductors at 100 amps or 115 amps?
