480-to-120V Transformer Odd Voltage

I have very rarely worked with anything above 240V before, but I recently was tasked with installing a 480V-to-120V AC transformer. I installed it as it indicated on the transformer's diagram (tied H2 and H3 together, left H1 and H4 available for two of the 480V source wires, tied X1 and X3 together to be my 120V Hot, and tied X2 and X4 together to be my 120V Neutral, made the ground common by connecting the existing ground from the 480V to the ground lug on the transformer). I connected the 480V Hot1 to H1 and Hot2 to H4.

On the output side, I'm getting 128V between Hot and Neutral, 112 between Neutral and Ground... But I'm also getting 180V between Hot and Ground. Does anyone have ideas why this might be? I did run wire through a conduit pipe, so it's possible a wire could have lost some of its sheath, gotten exposed, and touched the conduit. Wanted to see if anyone had another idea. Thanks!
 
I am closing this thread in accordance with forum rules. We are not allowed to assist a person who is not an electrician to perform their own electrical installation work. Engineers are certainly welcome to participate in this forum. I have learned a great deal from the other forum members over the past 20+ years. But please don't post DIY questions.
