Does the heating element itself have a thermal shutoff? Often heating elements are manufactured with a T/C sheath in the bundle for control and another for shutdown in case of over temp of the bundle (normally fixed to one of the elements). If you lose water flow for some reason, the temperature of the elements can get way out of line, even if the water where the control T/C is shows it is OK.

I would not focus on PQ. Even if the voltage were several times what it was rated at, the short period of time the heater is energized, coupled with the high thermal mass would tend to make this a non-issue.
