I have a project that is looking for the results from insulation resistance tests done on all the 600V cable feeders. We have completed all the tests but need to formalize onto a report. Can anyone direct me to where I can locate a sample test report form.

Have you thought about making a chart using Microsoft Excel? Label the columns with the tests performs and the rows identifying the cables tested with the results entered under each column. Organize your data buy making sketching first and then formalize if with excel.

Mine are all automated so can't help you but you can make one easy in excel, should have measured resistance, ambient tempature, tempature corrected resistance values, test voltage, and cable length/type.
