I've got an installation where a feeder is fed from a 225 amp, molded case breaker in a panelboard. The feeder then runs into another room and into an enclosure (not a disconnect switch) containing fuse blocks and fuses rated smaller than the breaker. Conductors from the fuses leave the enclosure and supply separate equipment a few feet away. The circuit breaker in the panelboard is readily accessible. But, the fuse enclosure has a screw cover and has no switch or disconnecting mechanism, so the circuit breaker is essentially the disconnecting means for these fuses.

Does the fuse enclosure have to meet 240.24(A), which requires overcurrent devices to be readily accessible?

None of the exceptions in 240.24(A)(1) through (4) apply.

If none of the exceptions apply seems like you answered your own question :D
You might also take a look at 240.40 and see if it's applicable.
