Adding service disc

I have a job coming up ( 2020 NEC) which requires us to add a breaker in a main lug 125 amp panel being used as service equipment.
Aka 6 handle rule.
The panel is 12/24 which would allow more the 6 two pole breakers.

The new service disc would be for a room addition feeder.(60 amp).
The existing meter will be changed from 100 to 200.
Which has a double lugged load side.

By adding this main would it be a violation of service mains in the same enclosure?

FYI, the other double lugged service conductors run inside to a main breaker panel.

I see that as an issue as well

Right now the outside existing main lug has two AC units and back feed PV in it.
I see the PV as a line side tap.
Which is feed from a PV system on a detached structure( garage) with a separate service.

There adamant this is ok to do.

There is nothing in the 2020 that would permit you to add a breaker. This was an oversight when 230.71(B) was changed in the 2020 code. The 2023 code added an exception to permit adding breakers to MLO service panels that were installed prior to the local adoption of the 2020 code.
Exception to (2), (3), (4), (5), and (6):
Existing service equipment, installed in compliance with previous editions of this Code that permitted multiple service disconnecting means in a single enclosure, section, or compartment, shall be permitted to contain a maximum of six service disconnecting means.

Many Main Lug Only panel contain a label that says something like 'Suitable for Use as Service Entrance when provided with not more than 6 devices. So it is up to the installer to make sure the installation complies with the appropriate codes.

The six-handle rule is about what you install, not what can be installed.
