AFCI per the 2014 NEC is required for all receptacles in kitchens. Does that mean that you need to have AFCI CB's and GFCI receptacles above counters and/or combination AFCI/GFCI CB's? for these circuits? Or is there an exception I'm missing?

You need AFCI at bc origin and GFCI in Kitchen- So AFCI brkrs and GFCI rec in kitchen is fine or you can just buy the AFCI/GFCI breakers for the areas where both are needed. There a few other options in 210.12 to provide afci, but some of those may not be applicable or some may even work for you but these 2 are less expense and trouble.

And no, there are no outs to AFCIs for kitchens in new construction per the 2014. The only way is if your AHJ lets you skate by not enforcing that part of 210.12.
