Am I correct in the following statement;
All receptacles in the dwelling must be AFCI?
Bathroom receptacles are GFCI only?
Garages are GFCI only?
Kitchen counter receptacles are combo GFCI/AFCI?

Does the line from the panel to the garage and bathroom need to be protected by a GFCI breaker of does the receptacle suffice?

You mention receptacles but the code states 125v outlets- these can also be lighting outlets. The 2014 NEC requires afci almost everywhere on 125V circuits except for garages, unfinished basements and bathrooms. Before the 2014 Kitchen and laundry rooms were also exempt.
