I was taking a test and they asked about hazardous energy, but I could not find a code reference.
Anybody know of a code that mentions lock out tag out procedures?

They may be talking about the hazard level calculation for arc flash. That information is in 70E, not NEC.

It is an FedOSHA standard and comprises more than electrical. State Plan States will have similar statutes.

Ditto on NFPA 70E. It contains specifics regarding LOTO, arc flash, etc.

Could it have simply been related to 110.16 (Arc Flash Hazard Warning)?

No, it wasn't that one. The question if I remember correctly was worded in a way that it was talking about safeguarding the worker from the equipment becoming energized while they were working on it. This was a state journeyman test, I'm not sure, but I didn't think state journeyman tests covered 70E.
