Appliance disconnect

Looking to get opinions what is required for a disconnect for a hydronic toe kick heater. The rating is less than 300va and the unit has a hi-off-low switch on the face of the unit. Also, if the unit were over 300va, what would be acceptable?
I'm having a tough time distinguishing between 422.31(A) and 422.31(B).

(A) The breaker can serve as the disconnect, such as a bath fan.
(B) The breaker can still be used. But if so, it must be with in sight of the disconnect. (Or lockable,110.25) Such as a 480V HVAC fan
