Is a refrigerator and microwave considered a fastened in place appliance load that be applied to the 75% demand factor for four or more appliances?

I would not consider a refrigerator fastened in place...but if its in the kitchen, it would be part of the small appliance circuit load.

I have included built-in microwaves in the appliance demand factor.

Refrigerator, no unless built-in custom.
Microwave, yes if actually built in (combo micro and range hood for example) rather than a countertop unit slid onto a framed shelf.

220.52(A) exception says the refrigerator can be excluded from the SABC calc. If this is excluded can I included in the appliance load?

I do not think so, unless you can figure out a way to fasten it in place. IMO fridges and washers should be allowed, but they are not. You can use WHs though.

220.53 Appliance Load ? Dwelling Unit(s). It shall be
permissible to apply a demand factor of 75 percent to the
nameplate rating load of four or more appliances fastened
in place
, other than electric ranges, clothes dryers, spaceheating
equipment, or air-conditioning equipment, that are
served by the same feeder or service in a one-family, twofamily,
or multifamily dwelling.

You don't NEED to include it, it is already included in the SABC loads.

The exception to 220.52 says that if you provide the refrigerator with an individual branch circuit, you don't need to figure additional load for that circuit. (Are you trying to use it to get to 4 appliances so you can apply the demand?)

Yes... lol
