Application of General Note 3 under Table 300.50

It is not clear if note #3 under table 300.50 applies to all columns of the table, specifically column #6. As it is listed as a General Note under the table I tend to feel it does apply but......
I could really use the additional 6" in a few locations on the site. I have a 4" concrete envelope around the duct bank so I could really gain 12" based on that note.

It specifically refers to notes 1, 2, and 3 as "general notes". I would take that as meaning it applies to all of the table. Note 3 only applies to "industrial establishments" so you might want to make sure you meet the definition of "industrial establishment", a term that I seem to recall is not defined in the NEC so might mean just about anything, or nothing.

I don't deal with UG MV cables so I can't say I have ever run across this before.
