Does anyone know what kind of clearance is required above this type of gear? I'm wondering what is required to cope with an arc flash that is directed out of the top of the gear?

If your switchgear is actually designed to vent the arc flash out of its top (i.e. listed as Arc Resistant), the manufacturer should provide you with the clearances required.
 
Our 38 kV arc resistant gear requires 48" of clear space above vented sections. Our plenum has a total of 7 top-vented sections on a 21 unit lineup. If you don't have that much headroom available they will duct the plenum outside instead of venting it upward - this would eliminate the 48" requirement. I don't have any experience with 5 kV or 15 kV arc resistant switchgear.
