Can someone help me understand the explanation given in the HB? I'm not a good typist so hopefully someone will have the CD to quote.

I'm looking through 250 and cannot find what is meant in 200.2(B). I may need a picture to understand the intent, sorry!!!
 
I believe it is saying that in most premise wiring there should be a grounded conductor brought in to the service.

Picture a panel with 2 neutral bars mounted directly to the enclosure. The enclosure itself cannot be used to establish continuity, there must be a bus or conductor between them.

Here is the quote, commentary in blue.

200.2(B) Continuity.

The continuity of a grounded conductor shall not depend on a connection to a metallic enclosure, raceway, or cable armor.

Section 200.2(B) requires the grounded conductor to connect to a terminal or busbar that is specifically intended for connection of grounded or neutral conductors. Some electrical distribution equipment used as service equipment has equipment grounding busbars that are separate from the grounded conductor busbar. The two are connected through the main bonding jumper. This section prohibits using the main bonding jumper and the metal enclosure of service equipment (or system bonding jumper for a separately derived system) as a means to connect a load side grounded conductor to the supply side grounded conductor as would be the case where a grounded conductor is terminated on an equipment grounding busbar. Such a connection results in the entire metal enclosure becoming a current-carrying conductor.
