Anyone know of an example of exceptions 1 and 2 be it picture or website?

Been trying to picture one.

Also, where does it say in the code that you can only have one NM cable per opening in a NM box?

I don't know of an article that restricts you to 1 cable per opening, however the listing or instructions might.

314.(17) exception refers to single gang boxes less than 4" x 2?", and the last sentence allows multiple cable entries in a single knockout opening.

Sheathed Cable Only.?? Clamps have been tested for securing only one
cable per clamp, except multiple section clamps are considered suitable for
securing one cable under each section of the clamp, each cable entering a
separate knockout.

I think that last sentence does it. From page 248 of the UL White Book
