Can service conductors primary or secondary pass under the slab of the building they serve. Example pad transformer in front of the building and the meter center in the rear.slab is 4 inches thick. I have an inspector telling me they have to hit the meter first
 
Not sure what primary and secondary service conductors are. Service conductors are “unfused” and there are restrictions on entering a building before hitting an OCPD. But service conductors under 2” of concrete are outside the building. A meter is not required by the NEC, your inspector is wrong
 
Primary to the padmount and secondary to the meter or the service point are typically the utilities discretion.

For your rebuttal, in the 2020 NEC, you are quoting 230.6 "Conductors to be considered outside the building" and 230.70(A)(1) where the service conductors are still outside of the building when they pass underneath the building.
