Article 250.146

When did the rule for article 250.146 Start the earliest addition of the NEC I have is 2002 and the rule is in there so how far back does the rule go for connecting the receptacle grounding terminal to a metal enclosure.

Sometime in the late 1960s to early 1970s we entered the modern era of equipment grounding for all receptacles, before that it was only required in certain areas.

This is from the 1965 and 1962 NEC:

250-74. Bonding at Grounding-Type Receptacles. Grounding continuity between a grounded outlet box and the grounding circuit of the receptacle shall be established by means of a bonding jumper between the outlet box and the receptacle grounding terminal.
Exception No. 1: When the box is surface-mounted, direct metal-to-metal contact between the device yoke and the box may be used to
establish the grounding circuit.
Exception No. 2: Contact devices or yokes designed and approved for the purpose may be used in conjunction with the supporting screws to
establish the grounding circuit between the device yoke and flush-type boxes installed in walls

1962 NEC:
250-74 Bonding at Grounding-type Receptacle. Where there is a likelihood that grounding continuity, especially at flush-type boxes, will not be maintained between a grounded outlet box and the grounding circuit of the receptacle through the mounting yoke, a bonding jumper shall connect the grounding terminal of the receptacle to the grounded box.

Something similar to the current form of the table first appeared in the 1968 NEC. You can see an additional 40 amp OCPD listed and the missing 300 amps from the current table.
