On a 4000 ampere electrical service, does the grounding electrode conductor need to be larger than the 3/0 (CU) conductor indicated in Table 250-66? If so, where is this referenced in the NEC?

It depends on the type of grounding electrode. the exceptions to table 250.66 are given in section 250.66
3/0 is the maximum code required grounding electrode conductor no matter how large the service. The design documents will often require larger GECs, but the code does not.
