Can anyone tell me if it is allowable to make an extension ring from a pull box ie, cut the back out and mount it over the existing flush mounted (in concrete) box? (The existing embeded box is 10" x 10").

Holes are permitted in pull boxes. There is no reference to the shape or size. Cutting an opening in the back, for entry, is a proper procedure.

I can't see anything wrong.

314.22 allows extension rings over flush mounted boxes, and there is nothing stated an extension ring cannot be fabricated. The one time I found myself in this position, the only comments I got from the AHJ were that the hole in the back of the surface box must be of appropriate size to acess any splices in the flush box, and that the surface box be mounted to the flush box, not soley the wall.

As long as it is secured well, and there are no issues with abrasion, you should not have a problem with this.
