Article 517.13 (A) & (B) requires a redundant ground for receptacles and fixed electrical equipment in patient care areas via a metallic conduit (or a cable having a metallic armory or sheath assembly) and a insulated copper equipment grounding conductor.

Is this redundant ground only required from the branch breaker to the receptacle or fixed electrical equipment? Or, is the redundant ground required in the panel feeder also?
 
Just the branch circuit.
Note; that if using a cable assembly the particular cable type must be recognized in article section 250.118.
